New patients are asked to arrive 15 minutes before the scheduled appointment time and have your insurance card and driver’s license available. For your convenience, our "Patient Forms" web page contains forms that you can print off, complete and bring to your appointment.
Appointment Cancellations
- As a courtesy you will receive an automated call to remind you of your scheduled appointment 48 hours in advance.
- To better accommodate the needs of our patients, we request a 48 hour notice if you are unable to keep a scheduled appointment.
- A $35 fee will be charged to your account for appointment cancellations less than 24 hours in advance.
