  • The amount for which you are responsible (any deductibles, copays, percentages or non-covered services) is required at the time of service.
  • If you do not have one of the plans the practice is contracted with, the total cost of your visit is required at the time of service.
  • As a service to our patients, we will bill insurance companies we are contracted with for services and allow them 45 days to render payment. After 60 days, you are responsible for the entire balance, paid-in-full.
